This questionnaire contains 14 sections. The table below shows the sections that will be asked of participants:

Fracture Injury Information Questions about the history of fracture injuries experienced by participants H. Questions for women Questions about information about women such as menstrual cycles, pregnancy, and early cancer screening tests I. Family History Questions about the information on family history and illness including siblings, parents, and children J. Physical Activity Questions about the record of the daily routine for any body movement for more than 10 minutes while working and doing activities at home and in fitness activities K. Soalan soal selidik mengandungi 14 bahagian. Berikut adalah bahagian yang akan ditanya kepada peserta:
Bahagian Keterangan A. Demografi Soalan maklumat peribadi peserta B. Latar Belakang Perubatan Soalan mengenai sejarah perubatan peserta yang disahkan oleh doktor C. Perubatan Komplimentari dan Alternatif (CAM) Rawatan alternatif yang diambil peserta D. Sejarah Pembedahan Soalan mengenai maklumat pembedahan yang pernah dijalani oleh peserta E. Sejarah Imunisasi Soalan mengenai maklumat tentang vaksinasi yang pernah diterima oleh peserta F. Pemindahan Darah Soalan mengenai maklumat pemindahan darah yang pernah diterima oleh peserta G. Maklumat Kecederaan Patah Tulang Sejarah kecederaan patah tulang yang pernah dihadapi peserta H. Soalan untuk wanita Soalan mengenai maklumat tentang wanita seperti kitaran haid, kehamilan dan ujian pengesanan awal kanser I. Sejarah Keluarga Soalan mengenai maklumat tentang sejarah dan penyakit keluarga termasuk adik-beradik seibu sebapa dan anak J. Aktivit Fizikal Rekod rutin harian bagi sebarang pergerakan badan melebihi 10 minit semasa bekerja, melakukan aktiviti di rumah dan aktiviti kecergasan K. Persekitaran dan Pekerjaan Soalan tentang kawasan perumahan dan pekerjaan L. Food Frequency Questionnaire
With the aim to elucidate the relationship between food consumption and non-communicable disease (NCD), The Malaysian Cohort project is implementing the food frequency survey. Through the survey, we are able to assess the total diet, such as the portion size information and its frequency about food and beverage consumption, of Malaysian.
The survey is conducted via a self-administrated Food Frequency Questionnaire (FFQ). Participants will be explained and briefed on the details of the questionnaire before completing it. Based on our experience, it takes around 30-60 minutes to finish a questionnaire.
At the end of the survey, participants will receive a copy of summary report prepared by our dietary system. The report consists of two main components:
(1) daily intake of macronutrients and
(2) suggestions on ideal amount of nutrient intake.Hopefully, though the survey, our participants will gain some insights on their food intake pattern.

Kajian Kekerapan Makanan
Dalam usaha memahami perhubungan antara pengambilan makanan/diet seseorang dan risiko penyakit, projek The Malaysian Cohort telahpun melaksanakan kajian kekerapan makanan. Kajian tersebut dilaksanakan melalui soal selidik kekerapan makanan (FFQ). FFQ merupakan satu set soal selidik yang mampu menilai pengambilan makanan kebiasaan seseorang. Hasil daripada FFQ dapat memberikan gambaran diet (purata sehari) seorang individu.
Penjelasan ringkas akan diberikan kepada peserta sebelum sesi mengisi FFQ. Secara puratanya, 30-60 minit diperlukan untuk melengkapkan FFQ tersebut.
Keputusan peserta akan dianalisis oleh sistem dietitik kemudian. Selepas itu, cetakan keputusan yang mengandungi:
(1)maklumat pengambilan makronutrien seharian
(2)cadangan pengambilan makanan akan diberikan kepada peserta.Melalui kajian tersebut, pihak kami mengharap peserta akan lebih memahami corak pemakanan mereka, lalu mengubahsuaikannya kepada yang lebih sihat.
