Any request for professional or online opinion and/or for further treatment or conditions will not be entertained unless the relevant referral letter and its associated documents are uploaded.

You may also use this form to upload your referral letter (referral for Faculty of Dentistry KL Campus, Hospital Canselor Tuanku Muhriz (HCTM) or Hospital Pakar Kanak-Kanak UKM (HPKK) without having to physically submitting your referral letter.

We will process your referral letter in the usual manner (subjected that the referral is relevant or within scope of OMFS practice) and you will be contacted for your appointment within 2-3 weeks (only for non-urgent case)
