[09/11/2020] Counter Operation at Centre For Educational Extension During Conditional Movement Control Order (CMCO)– 09 November to 6 December 2020

To all students,

Counter operations during the Conditional Movement Control Order (CMCO) are only open on weekdays and will be closed on weekends. Students are required to:

• Make an appointment via this link https://forms.gle/16LoxknmzURn7STUA – students will be notified by email for confirmation of date appointment

• Make an application via this link https://gerak.ukm.my/mohon/login

• Complete the Health Declaration Form and must submit it at the PKP’s counter [Form]

• Adhere to the guidelines and recommendations set by the Ministry of Health such as social distance, use of face masks and hand sanitizers.

Please take note that the number of students while dealing at the counter is limited to five (5) individuals at one time.
